Diseases affecting the gastrointestinal tract, which includes the organs from mouth to anus, along the gastrointestinal tract, are the focus of this specialty. New Patients

Suncoast Internal Medicine Consultants is always accepting new patients. Prior to contacting our office for an appointment please contact your insurance company and verify that the provider you are wanting to see is In-Network with your individual insurance plan. All new patient packets, medical records and a script and/or referral from the referring physician explaining reason for visit is needed prior to any appointments being schedule. All new patient packets, referrals, and medical records can either be mailed to the office, or faxed to (727) 593-9904.

Requests for Medical Records

SunCoast Internal Medicine Consultants is committed to quality patient care and requires certain medical records from other medical professionals. To have your medical records sent to our office, please print and complete the attached Authorization to Disclose Health Information request form. The completed request form should be mailed to our office.  Below are the medical records that will be needed for any new patient and/or follow up visit.

Medical Records within the past 6 months:

  • Laboratory Testing
  • CT Scans
  • Abdomen Ultrasounds, x-rays, and/or MRI
  • Swallow Study
  • Office visits

Medical Records regardless of when they were done:

  • Esophagogastroduodenoscopy (EGD)
  • Endoscopic Retrograde Cholangiopancreatography (ERCP)
  • Colonoscopy
  • Any Colon Surgeries
